We may collect personal identification information from 1 Customers to provide solicited service, 2 Information regarding changes in services or policies, 3 Assess the quality of services provided, 4 Uphold acquired responsibilities with customers, respond to a survey, 5 In compliance to official regulatory obligations, 6 and to keep a complete customer File. However, the Clinic may disclose personal information in case of explicit requirement from a federal administrative or law enforcement entity.
For the cases mentioned above we require the following personal data:
Name, Residence, electronic mail, CURP, RFC, Phone number, age, gender, marital status, place of birth and billing information. We also collect sensitive data according to the personal information protection law in possession of a private party:
Personal Medical Record, Family medical record, weight and body measurements.
These sensitive data will be solely used to provide the required service and the most efficient line of treatment.
Customer may be able to limit the use and disclosure of your personal data. Customer will be in every right to access, verify or cancel his/her personal data, revoke us consent to use them. The customer will be able to do so, by contacting us via Phone line or direct contact to the Personal Data Manager.
According to the Personal Data law, you as the holder of your own personal data, have the right to access the data we collect, and the handling measures we adopt. You are entitled to rectify if necessary; cancelation in case the information is excessive for the originally intended purpose. The Clinic, in compliance to the article number 32 of the Personal Data law, will be in the faculty to deny access to and to rectification or cancelation of the personal data, and may oppose against treatment of the latter. As for the stated on article 24 of the Personal Data law, The Clinic will not be in obligation to cancel personal data.
You can exercise your rights as fore mentioned by presenting a signed application at the headquarters of the representative or via electronic mail. For further information please address the Personal Information manager.
We inform that your personal data may be shared, stored, handled and transferred domestically and abroad by several employees within the corporation.
In this case your information may be shared with other companies within the group where the Clinic is a member of. This is done to facilitate the services provided by the Clinic regarding your specific treatment. If you do not express any inconvenient in this regard, it is understood you are in agreement by default.
As for the exceptions presented in article 37 of the Personal Data law we may not transfer or disclose your personal information to a third party without prior consent.
We adopt appropriate data collection, storage and processing practices and security measures to protect against unauthorized access, alteration, disclosure or destruction of your personal information, to warrantee confidentiality, integrity and availability.
If we communicate or share your personal information with our business partners and trusted affiliates for the purposes outlined above, we demand they comply with the security measures needed to protect your personal data and restricting their use to no other than the established end.
